About the CARE Collaborative
CPQCC will launch the CARE Collaborative in 2025!
Leveraging CPQCC’s expertise in quality improvement, we plan to address unmet health-related social needs, then disseminate our work widely to California’s NICUs as well as other state and national perinatal collaboratives. The proposed framework for local implementation begins with family voices, community partners and multidisciplinary NICU teams, then will proceed to selecting and adapting a screening tool, developing a community resource guide, and finally developing PDSAs to integrate practices within NICU workflows, increase the quality of referrals and improve resource navigation.
